Turning 40 does not cause human growth hormone (GH) to suddenly fall off a cliff. The change is more gradual. Growth hormone is released by the pituitary gland in pulses, with some of the largest pulses occurring during sleep, and both average secretion and the size of those pulses tend to decline as adults get older.
That decline is real, but it is also one of the most oversimplified stories in anti-aging marketing. Lower GH and IGF-1 levels are associated with several changes that become more common with age, including higher body fat and lower muscle mass. Association is not the same as proof that declining GH is the single cause—or that pushing GH upward will reverse normal aging.
For men over 40, the useful question is not “How do I get my HGH back to age 20?” It is: what actually changes, what can I influence safely, and when does a possible hormone problem deserve medical testing?
What happens to growth hormone after 40?
Growth hormone secretion normally decreases across adulthood. The Endocrine Society’s scientific statement on hormones and aging notes that the decline is seen largely in the amplitude of GH secretory episodes, while circulating IGF-1 also tends to decline in parallel. This age-related pattern is sometimes called the somatopause.
That does not mean every 45-year-old has “low HGH” in the medical sense. Normal reference ranges change with age, and true adult growth-hormone deficiency is a clinical disorder that is usually associated with pituitary or hypothalamic disease, injury, surgery, radiation or childhood-onset deficiency. It is not diagnosed simply because someone is tired, gaining belly fat or recovering more slowly from workouts.
Normal age-related decline in GH is not the same thing as diagnosed adult growth-hormone deficiency. The treatment standards are very different.
GH and IGF-1: why both get discussed
GH is released in pulses, which makes a random blood measurement difficult to interpret. Growth hormone also stimulates production of insulin-like growth factor 1 (IGF-1), much of it from the liver. IGF-1 is more stable in the bloodstream and is often part of the medical evaluation when GH disorders are suspected.
Even IGF-1 is not a DIY diagnostic test. Age, nutritional status, liver function and other factors can influence it. Endocrine guidelines note that confirmation of adult GH deficiency generally requires appropriate stimulation testing unless there is a well-established structural or genetic reason that makes deficiency highly likely.
This matters because supplement advertising often treats “higher IGF-1” as automatically synonymous with looking younger, building muscle or losing fat. Biology is more complicated than that. A biomarker can move without producing every outcome people associate with the hormone pathway.
Can declining GH explain how you feel after 40?
Some features seen in true adult GH deficiency—changes in body composition, reduced exercise capacity and lower quality of life—overlap with complaints many otherwise healthy middle-aged adults have. That overlap is exactly why self-diagnosis is unreliable.
Poor sleep, sleep apnea, low physical activity, calorie excess, alcohol, medication effects, thyroid disorders, depression, low testosterone, anemia and many other issues can affect energy, recovery and body composition. If something has changed dramatically, it makes more sense to investigate the cause than to assume GH is responsible.
One particularly important confounder is body fat. The Endocrine Society notes that obesity, especially visceral fat, is associated with reduced GH secretion. In other words, the relationship can run in more than one direction: aging and lifestyle influence body composition, and body composition itself can influence the GH axis.
Sleep, exercise and the GH axis
Growth hormone is tightly connected with sleep physiology. A substantial pulse often occurs around the beginning of deep sleep. That makes sleep relevant, but it does not mean a “sleep hack” can reliably restore youthful hormone levels. The practical takeaway is simpler: chronic poor sleep works against healthy aging in many ways, regardless of whether you ever measure GH.
Exercise is similarly important. High-intensity and resistance exercise can produce acute GH responses, but the long-term value of training after 40 goes far beyond chasing a temporary hormone spike. Progressive resistance training helps preserve strength and lean mass; aerobic work supports cardiovascular health and work capacity; both can help with body composition and metabolic health.
If I were building a healthy-aging plan around the GH axis, I would put consistent sleep, resistance training, adequate protein, sensible calorie intake and maintaining a healthy waistline ahead of any supplement. They have broader evidence and benefits that do not depend on a single hormone theory.
What about prescription HGH?
Prescription recombinant human growth hormone is a real medical treatment. For adults with confirmed GH deficiency, guidelines recognize potential benefits in areas such as body composition, exercise capacity, skeletal integrity and quality of life. That does not make prescription HGH an evidence-based anti-aging treatment for otherwise healthy adults.
The Endocrine Society specifically advises against giving GH to older adults merely because IGF-1 is low for age in the absence of pituitary or hypothalamic disease. Mayo Clinic likewise notes that research does not support HGH as a way for healthy adults to regain youth and energy, and treatment can cause adverse effects.
So when a supplement is called an “HGH releaser,” keep the categories separate. A dietary supplement is not prescription somatropin, and evidence about medically prescribed GH cannot simply be transferred to an oral supplement.
Do natural HGH releaser supplements work?
This is where Vitamin Hippo will spend a lot of time separating interesting evidence from exaggerated conclusions. Amino acids such as arginine and compounds such as GABA have been studied in relation to acute GH responses under particular conditions. But an ingredient affecting a hormone marker in a small experiment is not the same as proving that a finished supplement reverses aging.
One reason I continue to follow GenF20 Plus is that I used it myself years ago and the finished product has a published randomized trial. Even there, the evidence needs nuance: the full study population did not show a statistically significant between-group difference on the primary IGF-1 analysis, while a baseline-adjusted analysis in participants 40 and older did. The trial did not establish significant advantages for outcomes such as body fat, lean mass, energy, libido or sleep quality over 12 weeks.

When should you talk to a doctor?
Being over 40 by itself is not a reason to request HGH treatment. But if you have a history of pituitary disease, brain surgery or radiation, significant head injury, known childhood GH deficiency, or symptoms occurring alongside other pituitary hormone problems, medical evaluation is appropriate.
A clinician can decide whether IGF-1 or formal stimulation testing makes sense and can look for more common explanations for fatigue, body-composition changes or poor recovery. This is especially important because legitimate hormone disorders deserve proper treatment, while unnecessary hormone use can create risk without solving the actual problem.
My bottom line on growth hormone after 40
Growth hormone and IGF-1 generally decline as we age. That is established physiology. What is not established is the idea that this decline is a disease in every healthy adult over 40 or that restoring youthful GH levels is a proven anti-aging strategy.
I think the useful middle ground is to understand the pathway without becoming obsessed with it. Protect sleep. Train. Maintain muscle. Manage body fat. Eat well. Investigate meaningful symptoms rather than guessing. And if you choose to experiment with a legal dietary supplement, judge it as a supplement—with realistic expectations—not as a substitute for prescription HGH.
