For a lot of men, 40 is when testosterone suddenly becomes part of the conversation. Recovery feels slower, body fat is easier to gain, sleep may be less forgiving, and the internet is ready with one explanation: your testosterone is crashing.
The reality is less dramatic—and more useful. Testosterone does tend to decline across adulthood, but age alone does not tell you whether your level is low, whether a symptom is hormonal, or whether treatment is appropriate. In 2026, the Endocrine Society reiterated that hypogonadism should be diagnosed from both compatible symptoms and consistently low, accurately measured testosterone, not from age or a single blood test.
So if you are wondering what testosterone after 40 really looks like, start with the distinction that marketing often skips: normal aging is not automatically testosterone deficiency.
What actually changes with testosterone after 40?
Testosterone production is controlled by a feedback system involving the hypothalamus, pituitary gland and testes. Levels vary between men and within the same man from day to day. They also follow a daily rhythm, generally peaking earlier in the day.
Population studies show an age-related decline in testosterone, but the slope is not identical for everyone. Body weight, chronic illness, medications, sleep, metabolic health and general fitness can all influence the picture. That is why two healthy 48-year-old men can have different testosterone results without either number automatically representing disease.
The Endocrine Society's patient guidance describes a gradual decline beginning in adulthood, while a recent review of male aging emphasizes that sleep disruption, metabolic dysfunction and hormonal changes interact in both directions. That matters because some potentially modifiable factors can pull testosterone down independently of age.
Not “How do I get the testosterone of a 20-year-old?” but “Do I have persistent symptoms, are my levels actually low on proper testing, and are there reversible factors worth addressing?”
What does low testosterone feel like?
Symptoms associated with testosterone deficiency can include reduced sexual desire, fewer spontaneous erections, erectile difficulties, lower energy, reduced muscle mass, mood changes and—in some cases—unexplained anemia or reduced bone density. The problem is that several of those symptoms are common for reasons that have nothing to do with testosterone.
Fatigue can come from sleep apnea, anemia, thyroid disease, depression, medication effects or simply chronic sleep debt. Erectile dysfunction can be vascular, neurological, psychological or medication-related. Changes in body composition can reflect diet, activity and aging. How testosterone should actually be tested
A random afternoon test is not the ideal way to settle the question. Current Endocrine Society guidance recommends confirming suspected hypogonadism with at least two early-morning, fasting testosterone measurements, interpreted alongside symptoms. If results are low, clinicians may investigate whether the problem is primarily testicular or related to the pituitary/hypothalamic signaling system.
Total testosterone is usually the starting point. Free testosterone—the fraction not tightly bound to proteins—can add context in selected situations, particularly when binding proteins may make total testosterone harder to interpret. The important point is that a lab number should be interpreted in context rather than used as an online score of masculinity or vitality.
A threshold near 300 ng/dL is commonly used clinically, but laboratory methods and professional guidelines differ. One result near a cutoff is not a diagnosis. Symptoms, repeat testing, assay quality and the clinical picture all matter.
Body fat, weight loss and testosterone
One of the strongest practical connections after 40 is between metabolic health and testosterone. The Canadian Urological Association guideline describes a bidirectional relationship between obesity and testosterone: excess adiposity can suppress the hormone axis, while low testosterone can coexist with unfavorable body composition.
For men carrying excess weight, sustainable weight loss can increase testosterone levels. That does not mean every man with low T can “diet his way out” of true hypogonadism. It means body composition is one of the first reversible factors worth considering before assuming age alone is responsible.
Crash dieting is not the answer either. Severe calorie restriction and poor nutrition can temporarily suppress reproductive hormones. The boring middle—adequate protein, a manageable calorie intake, mostly nutrient-dense foods and a body weight you can maintain—is more useful than a supposed testosterone superfood.
Sleep may matter more than most testosterone hacks
Testosterone production is closely linked with sleep. Persistent sleep restriction and sleep disorders can interfere with the normal overnight hormone rhythm. The Canadian guideline notes evidence that sleep restriction can reduce testosterone, while newer research continues to treat sleep, metabolism and male hormonal aging as interconnected rather than separate problems.
This is one area where a smaller men's-health publication, Hone Health's medically reviewed sleep-and-testosterone guide, provides a useful practical overview alongside the primary research. Their commercial relationship with hormone testing/treatment is worth keeping in mind, but the article links its claims to published studies.
If you snore loudly, stop breathing during sleep, wake unrefreshed despite enough time in bed, or have significant daytime sleepiness, consider discussing sleep apnea with a clinician. Treating an underlying sleep disorder is more meaningful than trying to “boost T” around it.
Resistance training: valuable even without a permanent T spike
Strength training is one of the best things a man over 40 can do for muscle, bone, function and metabolic health. But its value should not be reduced to whether it permanently raises resting testosterone. A meta-analysis in older men found resistance training did not significantly increase basal testosterone overall, even though exercise can create acute hormonal responses.
That is not a reason to skip the weights—it is a reason to use the right outcome. Getting stronger, preserving lean mass, improving insulin sensitivity and maintaining physical capacity are wins whether your resting testosterone changes by 0 or 100 points.
Nutrition and common “testosterone nutrients”
Zinc, magnesium and vitamin D frequently appear in testosterone supplements. Correcting a true deficiency can be sensible, but taking more of a nutrient when you already have enough is not guaranteed to push testosterone higher. This distinction is lost in a lot of supplement advertising.
Some botanicals have more interesting evidence. Reviews have identified ashwagandha and fenugreek extracts as ingredients with some positive human data in particular populations, while a broader systematic review concluded that most proposed testosterone boosters fail to reliably increase total testosterone. That mixed evidence is exactly why we evaluate formulas ingredient by ingredient rather than treating “natural” as proof of effectiveness.
Where testosterone supplements fit
A testosterone-support supplement is not testosterone replacement therapy and should not be used to self-treat diagnosed hypogonadism. At best, a formula may provide nutrients or botanical extracts with evidence for supporting testosterone in certain circumstances. At worst, it is an expensive label built around weak or irrelevant studies.

What about TRT after 40?
Testosterone replacement therapy can be appropriate for men with confirmed hypogonadism. It is not an anti-aging vitamin. Treatment requires an accurate diagnosis, discussion of benefits and risks, and ongoing monitoring. Fertility is especially important: exogenous testosterone can suppress sperm production, so men planning fertility need to discuss alternatives with a specialist.
The Endocrine Society also stresses that long-term safety questions remain and that screening and monitoring matter. If you are symptomatic and repeatedly test low, that is a medical conversation—not something a supplement review should decide for you.
A sensible plan if you're over 40 and concerned
Start with the basics you can actually measure: sleep quality, waistline, training consistency, nutrition, medication changes and whether symptoms are persistent or merely a bad week. If low libido, erectile changes, unexplained fatigue or loss of strength continue, bring them to your healthcare provider and ask whether properly timed testosterone testing makes sense.
If testing is normal, that is useful information too. It pushes the investigation toward other causes instead of chasing a hormone number that was never the problem.
Bottom line
Testosterone after 40 deserves attention, not obsession. Levels often drift downward with age, but the clinically important issue is whether a man has symptoms plus consistently low measurements. Sleep, excess body fat, illness and other modifiable factors can influence the picture, while legitimate hypogonadism deserves proper medical evaluation.
